Vets Diagnosed Gum Disease in One Dog in Eight. Studies That Look Under Anaesthetic Find Far More.

A study of 22,333 UK dogs put the recorded one-year prevalence at 12.5% — while detailed examinations of anaesthetised dogs report between 44% and 100%.

Plain-English summary

Two credible numbers describe gum disease in dogs, and they are very far apart.

In a study of 22,333 UK dogs under ordinary primary veterinary care, periodontal disease was diagnosed in 12.52% over the course of a year — about one dog in eight. But a review of the wider literature notes that when dogs are anaesthetised and their mouths examined properly, studies report prevalences between 44% and 100%.

Both are honest measurements. The distance between them is the interesting part: it is the space where the disease exists but has not yet been found.

What the study found

The researchers drew a random sample of 22,333 dogs under primary veterinary care in 2016 from the VetCompass database and modelled risk factors with random-effects multivariable logistic regression.

The one-year period prevalence of a recorded periodontal disease diagnosis was 12.52% (95% CI 12.09–12.97).

Eighteen breeds showed increased odds compared with crossbred dogs. The highest were Toy Poodle (OR 3.97, 95% CI 2.21–7.13), King Charles Spaniel (OR 2.63, 95% CI 1.50–4.61), Greyhound (OR 2.58, 95% CI 1.75–3.80) and Cavalier King Charles Spaniel (OR 2.39, 95% CI 1.85–3.09). Four breeds showed reduced odds.

Two conformation patterns emerged: brachycephalic breeds had 1.25 times the odds of periodontal disease compared with mesocephalic breeds (95% CI 1.11–1.42), and spaniel types had 1.63 times the odds of non-spaniel types (95% CI 1.42–1.87).

Size ran the opposite way to intuition. Increasing adult bodyweight was associated with progressively decreasing odds — small dogs carried the higher risk.

For the comparison number, the 2020 review by Wallis and Holcombe reports that primary-care practices, where diagnosis rests largely on visual assessment of conscious dogs, find an average prevalence of 9.3% to 18.2% — consistent with this study. Detailed examinations of anaesthetised dogs report 44% to 100%.

What this study cannot tell you

The headline figure counts diagnoses recorded in clinical notes, not disease present in mouths. A dog with early periodontal disease that nobody looked closely at is counted as not having it. This is a study of what primary care detects.

That also means the breed odds could partly reflect who gets examined. Owners of small breeds and of breeds known for dental problems may present more often or have dental checks raised more readily, which would inflate apparent risk in exactly those groups. A record-based study cannot separate true incidence from detection intensity.

It describes UK dogs in 2016 and does not evaluate any prevention strategy — nothing here tests brushing, diet, chews, or professional cleaning frequency.

And the 44–100% comparison range is very wide, drawn from studies with different populations and grading methods. It is the right order-of-magnitude context for the detection gap; it is not a precise figure.

What pet owners should track

Mouths are easy to ignore because they are unpleasant to look at and dogs rarely complain:

  • Breath, and specifically whether it has changed — worsening breath is more informative than breath being bad
  • Whether your dog has become reluctant to have its head or muzzle handled
  • Chewing on one side, dropping food, or slowing down at meals
  • Visible tartar, redness at the gumline, or a tooth that looks loose or discoloured — a phone photo is far better than a description
  • Note your dog's risk group honestly: small breed, spaniel type, flat-faced, or simply older

Bring these up rather than waiting to be asked, especially if your dog is in one of the higher-odds groups.

Why do the two numbers differ so much?

Because they measure different things. The 12.52% is how often periodontal disease was actually diagnosed and recorded during a year of ordinary primary care, where assessment is largely visual and the dog is awake. The 44-100% range comes from detailed examinations of anaesthetised dogs, where the whole mouth including below the gumline can be assessed properly. The gap is a detection gap, not a disagreement.

Which dogs are most at risk?

In this study, 18 breeds had increased odds compared with crossbred dogs. The highest included Toy Poodle (OR 3.97), King Charles Spaniel (2.63), Greyhound (2.58) and Cavalier King Charles Spaniel (2.39). Spaniel types had 1.63 times the odds of non-spaniel types, and brachycephalic breeds 1.25 times the odds of mesocephalic ones.

Are small dogs really worse off?

That is what this data shows. Increasing adult bodyweight was associated with progressively decreasing odds of periodontal disease — so the smaller the dog, the higher the risk, which is the opposite of what many owners assume.

Is bad breath enough to go on?

It is worth reporting, but it is not a diagnosis and its absence is not reassurance. This study measured recorded diagnoses and breed risk; it did not evaluate any owner-observable sign as a screening test. Periodontal disease is assessed by a veterinarian, and the thorough version of that assessment happens under anaesthetic.

What is the practical takeaway?

If your dog is in a high-risk group — small, or one of the predisposed breeds, or simply getting older — ask specifically about a dental assessment rather than waiting for it to be raised. The detection gap in this data exists partly because a conscious visual check can only see so much.
