In 15,000 Dogs, the Odds of Cognitive Dysfunction Rose 52% With Each Additional Year of Age

The largest cohort yet examined also found a striking link with inactivity — one that is far easier to misread than it looks.

Plain-English summary

Older dogs change. They sleep differently, get confused in familiar rooms, greet people less, or start pacing at night. Owners very often read this as ordinary ageing — and sometimes it is, and sometimes it is a recognised condition called canine cognitive dysfunction.

A 2022 study used the Dog Aging Project's nationwide cohort to look at this in more than fifteen thousand companion dogs, by far the largest group examined. Age dominated: each additional year raised the odds of cognitive dysfunction by about half again, even after accounting for everything else measured.

It also found a large association with inactivity. That number is the one most likely to be misused, and the study's design means it genuinely cannot tell us which way the arrow points.

What the study found

The researchers applied a minimally modified cognitive dysfunction rating scale, developed by earlier researchers, across 15,019 dogs enrolled in the Dog Aging Project. Associations with canine characteristics were examined with univariable and multivariable logistic regression and receiver operating characteristic analysis.

Controlling for all other characteristics measured, the odds of cognitive dysfunction increased 52% with each additional year of age.

Among dogs of the same age, health status, breed type and sterilisation status, the odds were 6.47 times higher in dogs described as not active compared with those described as very active.

Controlling for age, breed type, activity level and other comorbidities, dogs with a history of neurological, eye, or ear disorders had higher odds of cognitive dysfunction.

The researchers also found that lifespan quartile — an estimate based on the dog's weight — discriminated well between dogs scoring positive and negative on the scale, and suggested weight-based lifespan quartile estimation could help veterinarians decide whom to screen.

What this study cannot tell you

The activity finding deserves to be stated plainly: this analysis is cross-sectional. Activity level and cognitive status were captured at the same moment. A 6.47-fold association is perfectly compatible with cognitive decline reducing activity, with inactivity contributing to decline, or with a third factor such as pain or arthritis driving both. The study establishes that these things travel together. It does not establish that exercise protects the brain, and it should never be presented that way.

Cognitive status here is a score on an owner-completed rating scale, not a clinical diagnosis. No imaging or post-mortem confirmation was involved. That is the only realistic approach at this scale, but it means the outcome measured is reported behaviour.

The sensory finding carries its own trap. A dog with failing eyes or ears may score higher on a behavioural scale simply because it cannot navigate or respond — which looks like disorientation without being cognitive disease.

Finally, the Dog Aging Project is a volunteer cohort. Owners who enrol their dogs in a longitudinal research project are not a random sample of dog owners.

A 2023 author correction was published for this paper. It corrected the consortium author list only; none of the findings changed.

What pet owners should track

Behavioural change in an older dog is worth writing down as it happens, because it arrives gradually enough to be absorbed as the new normal:

  • Disorientation in familiar places — standing in corners, going to the hinge side of a door, seeming lost in a room they have always known
  • Changes in how your dog greets or seeks out people and other animals
  • Sleep changes, especially restlessness or pacing at night against more sleeping by day
  • House-soiling in a dog that was reliably clean
  • Changes in overall activity — and note whether stiffness or reluctance to move seems to be part of it, since pain and mobility problems are separate things a vet will want to distinguish
  • When each change started, and whether it has progressed

Mention sight and hearing explicitly. Sensory loss is common in older dogs, is treatable in some cases, and can look very much like confusion.

Frequently asked questions

Will keeping my dog active prevent cognitive decline?

This study cannot answer that, and it is the most important thing to understand about it. The 6.47-fold association between inactivity and cognitive dysfunction was measured at a single point in time, so it cannot show which came first. Cognitive decline itself makes a dog less active — that explanation fits the same data equally well. Exercise is good for older dogs for many reasons, but this study is not evidence that it prevents dementia.

How much does age alone matter?

A great deal. When all other measured characteristics were held constant, the odds of cognitive dysfunction increased 52% with each additional year of age.

My dog is losing his sight and hearing. Does that count?

It complicates the picture. Dogs with a history of neurological, eye, or ear disorders had higher odds of cognitive dysfunction in this study. But a dog that cannot see or hear well may bump into things, fail to respond, or seem lost — behaviours that look like cognitive decline on an owner-completed scale without necessarily being it. This is a reason to have sight and hearing assessed rather than assume dementia.

Was cognitive dysfunction actually diagnosed in these dogs?

No. Status came from a rating scale completed by owners, not from a veterinary diagnosis, brain imaging, or post-mortem confirmation. That is a reasonable approach for a cohort of this size, but it measures reported behaviour rather than confirmed disease.

What should I do with this?

Treat behavioural change in an older dog as something worth recording and raising, rather than something to file under normal ageing. The decision about what it means belongs to your veterinarian, who will want to rule out pain, sensory loss, and other medical causes first.
