Plain-English summary
Hyperthyroidism is one of the most common hormonal disorders in older cats, and weight loss is one of its signatures. What this 2016 study of 462 untreated cats shows is how easily that weight loss hides.
The cats had lost about a kilogram, on average, compared with what the record said they weighed one to two years earlier. But only about a third of them actually looked thin. The rest had shed a substantial amount of weight and still landed in an ideal or overweight body condition — because they had started out heavier.
Underneath that, something less visible was happening. More than three-quarters had lost muscle mass. The number on the scale understated it; the way the cat looked understated it further.
What the study found
Four hundred and sixty-two cats with untreated hyperthyroidism had their body composition evaluated: body weight, body condition score, and muscle condition score. A subset of 117 were reassessed three to twelve months after treatment, once euthyroid.
Median body weight before treatment was 4.36 kg (IQR 3.5–5.2), significantly lower than the premorbid weight of 5.45 kg (IQR 4.6–6.4) recorded one to two years before diagnosis.
Despite that, only 154 cats — 35.3% — were assessed as thin or emaciated. Muscle told a different story: 357 cats, or 77.3%, had lost muscle mass.
After treatment, the subset that was reassessed showed significant improvement across the board: median body weight rose from 4.1 kg to 5.0 kg, median body condition score from 3/5 to 3.5/5, and median muscle condition score from 2/3 to 3/3. But recovery was incomplete — mild-to-moderate muscle wasting persisted in 45% of treated cats.
The authors' conclusion draws the two threads together: most hyperthyroid cats lose weight while maintaining an ideal or overweight body condition score, with only a third being underweight, and that weight loss is associated with muscle wasting affecting more than 75% of them.
What this study cannot tell you
This describes cats who already had a hyperthyroidism diagnosis. It did not follow healthy cats to see whether weight tracking catches the disease earlier, and it did not test weight change as a screening test. Nothing here establishes that a home scale detects hyperthyroidism.
Weight loss and muscle loss are also not specific to this disease. Chronic kidney disease, diabetes, dental disease, cancer and simple ageing can all reduce weight and muscle in older cats. A falling weight is a reason to investigate, not an answer.
The premorbid weights came from existing clinical records taken one to two years earlier, not from a standardised research protocol — so the exact interval and measurement conditions varied between cats.
And the post-treatment findings rest on 117 of the original 462 cats, reassessed over a window as wide as three to twelve months. That is a useful signal about recovery, not a precise account of it.
What pet owners should track
The practical lesson is that a single weight is nearly meaningless and a weight history is genuinely valuable:
- Weigh your cat on the same scale and note the date — every one to two months is enough for an adult cat
- Keep the old numbers. The comparison in this study depended entirely on a weight recorded one to two years earlier
- Note appetite alongside weight. An increased appetite together with weight loss is a combination worth mentioning specifically
- Note changes you can feel rather than see — the spine, hips and shoulder blades becoming more prominent under your hands, even while your cat still looks well
- Record the body condition and muscle condition scores your veterinarian assigns, since these are separate assessments and the muscle one is easy to overlook
A cat that has dropped from 5.4 kg to 4.4 kg has lost close to a fifth of its body weight. Very few owners would perceive that by looking.
